(Solution) NR 451 Discussion Week 5: Interprofessional Collaboration

NR 451 RN Capstone Course

Planning for our patients during times of transitions (for example: hospital to home, home to rehabilitation facility) involves collaboration with a number of healthcare professionals. Please address the following questions:

  • How does your facility promote interprofessional collaboration during times of patient transitions?
  • What is the role of the nurse in patient transitions?
  • What gaps can you identify in this process related to quality of care? (If you are not currently in practice, please use a previous role or clinical experience in your answers.)

Transitioning patients from hospital to another hospital, or from hospital to home, or from hospital to a rehabilitation facility requires collaboration among different healthcare professionals so as to ensure that the needs of the patient and those of their families are taken into consideration, and everything is done for the best interest of the patient. Transition care is important in healthcare because it defines a broad range of time-limited services that are designed to ensure that there is healthcare continuity, promote the safe and timely transfer of patients, and to avoid preventable poor outcomes among at-risk populations (Melby, Brattheim and Hellesø, 2015).

For instance, it is the role of nurses to interact with the patients or their families during transition to ensure that they are comfortable with the idea. During these interactions, the nurse gets to learn the information deemed critical for successful transition….please click the purchase button below to access the entire solution at $5
