Purpose

This week’s graded topics relate to the following Course Outcomes (COs).

The Assignment

This week you have your choice of three discussion topics! Select the one that most interests you and answer the corresponding questions completely.

Remember to reference both the book or lesson, and an outside scholarly source.

Option #1:

You are the nurse assessing an Orthodox Jewish client with peptic ulcer. The client is strictly religious and refuses to eat the food provided at the health care facility.

  1. Describe how you would further assess and provide care for this client.
  2. What steps could you take to increase your cultural competence, if you were not familiar with this faith?

Option #2

You are the nurse caring for a client with Crohn’s disease. The client believes he is being punished by God. The client is spiritually distressed and cannot come to terms with the illness.

Spiritual distress is a disruption of one’s religious beliefs. A patient exhibiting spiritual distress loses hope, questions their own religious belief system and may live a lonely life or feel a lack of comfort and strength. As a nurse, it is crucial to assess for patients in this type of distress.

Sometimes people just need to be heard, vent, or even sit in silence with someone else who is nonbiased and judgmental, such as a nurse.

Option #3 

Describe a time in your clinical nursing practice when you have cared for a client of

cultural, religious, or spiritual practices different from your own.

  1. What were some of the challenges you faced caring for this client?
  2. What steps did you take (or could you have taken) to ensure the patient received culturally/spiritually competent care?

SOLUTION  

  1. How would you respond to this client?

First, I would listen to what the patient tells me, explains how they feel, and take the time to learn about their religious beliefs. I would listen to them with intent and remain nonjudgmental. It isn’t easy for a patient to open up in such a vulnerable and emotional state. I would allow adequate time for the patient to respond, express themselves, and make the environment as comfortable as possible. I would ask them if there is anything I can do to help them access any resources that would help them, if it would be helpful to speak to a chaplain or spiritual leader, are there any aspects of your spiritual practice that they find most helpful for them? What aspects of spirituality or spiritual practices do they find most helpful for them personally? For example, prayer, meditation, reading scripture, attending religious services, listening to music, hiking, communing with nature (Weber & Kelley, 2018).

  1. What are some identified risk factors for spiritual distress, and recommended interventions?
